Length runs from the PSU end to the first plug. Adapters that plug into other cables are left out. Use 16-pin PCIe when the maker names a 16-pin GPU cable without saying 12V-2x6 or 12VHPWR. A cable from a 12V-2x6 socket on the PSU to 8-pin plugs is a cable, not an adapter, and counts as PCIe 8-pin.
